Arizona Residents: Cigna, Optum, BCBS, and Aetna. Self pay/Cash

Initial psychiatric evaluation [45-60 minutes] | $300
Follow up [20-30 minutes] | $150
Minimum of 24 hours notice to avoid a cancelation/no show fee of $200 for a missed initial evaluation and $100 for a missed follow-up visit.
