Matrix Mental Health

Are you getting ready to enlist?

Fast, Professional, and Confidential Assessments for Enlistment

Are you or someone you know preparing to enlist in the U.S. military but facing challenges due to past mental health diagnoses or treatment history? We provide comprehensive, independent psychiatric evaluations to help assess fitness for service and assist in the waiver or MEPS clearance process.

Many recruits are surprised to learn that the military may require a mental health evaluation as part of the enlistment process—especially if there is a history of therapy, medication, or psychiatric diagnosis. If you’ve received mental health treatment in the past, you will likely be required to complete an evaluation before moving forward. We’re here to make that process fast, professional, and hassle-free.

Who Needs an Evaluation?

You may need a mental health evaluation if you have a history of:

  • Depression, anxiety, PTSD, or ADHD
  • Counseling or therapy during high school or college
  • Medication for mental health (past or current)
  • Psychiatric hospitalization or ER visit
  • Suicide ideation, self-harm, or behavioral concerns
Even if these issues were minor or in the past, the military will often request independent documentation to determine your fitness for service.

What We Provide

  • Independent psychiatric evaluations from a licensed PMHNP
  • Same-week virtual appointments across AZ, OR, CO, and NV
  • A detailed report addressing military standards and MEPS requirements
  • Clear, unbiased documentation to support waiver or enlistment efforts
We collaborate closely with recruits and recruiters to ensure everything is submitted correctly and quickly.

Pricing

We charge a flat fee of $300 due upon booking of the appointment.
